Serious Case Review Overview Sub-group

Role
To oversee and quality assure all SCRs undertaken by the CSCB

Terms of Reference
1. On receipt of information as to the circumstances of a child’s death to consider whether the criteria set out in Chapter 8, paragraphs 8.11 and 8.12 of ‘Working Together to Safeguard Children March 2010) for a SCR are met.

2. To make recommendations to the Chair of the Board, who will make the final decision, as to whether an SCR should be convened.

3. Where the decision made by the Chair is to convene such a review then the Group should also make recommendations as to the following:

• the scope of the review process.

• terms of reference for the review.

• the composition of the specific SCR Panel including an Independent Chair, author and Panel members that reflect all relevant agencies.

4. Where such Reviews are convened, to receive reports from the Chair of each individual Serious Case Review Panel convened and to ensure progress of the Review in line with agreed timescales.

5. To receive completed reviews and present these to the Safeguarding Board for endorsement.

6. Following agreement of Action plans, to address any lessons learnt and monitor agency actions to the point of completion, reporting on progress quarterly to the Board.

Frequency of Meetings
At least quarterly in order to report to the Board and as and when necessary to enable prompt consideration of any child deaths referred by agencies.

• Calderdale Council’s Children and Young People’s Directorate- Children’s Social Care.

• Calderdale Council’s Children and Young People’s Directorate.

• Calderdale PCT/ Safeguarding Team

• West Yorkshire Police Force/ Child and Public Protection Unit.

• CSCB Safeguarding Manager

• CSCB Minute Taker

NB. The Chair must be a member of the Board.
