Serious Case Reviews
Role
To consider whether to hold a
Serious Case Review on
the death of a child where abuse or neglect are
factors, or where there are serious concerns
regarding inter-agency working where a child suffers
potentially life threatening concerns, serious
impairment of health or development, or particularly
serious sexual abuse.
Terms of Reference
At the request of the CSCB Chair to decide whether
the criteria (Chapter 8, paragraphs 8.5 and 8.6 of
‘Working Together to Safeguard Children’ 2006) for a
SCR are met.

To determine the scope of the review process.

To draw up clear terms of reference.

To identify the composition of a SCR Overview Panel including an Independent Chair and Panel members that reflect all relevant agencies.

To make recommendations to the CSCB Chair who will have the final decision in relation as to whether or not to hold a SCR.

Report to the CSCB Chair via the Safeguarding
Manager

NB. The Chair must be a member of the Board.
NB The SCR sub-group can co-opt appropriate members (e.g. Designated doctor/ nurse, Local Authority Solicitor) to assist with its business.
